Outreach Support inclusive of domiciliary care, live-in carers. We involve the young person and those who know them well in the design and delivery of their support, and place each young person at the center of decision-making.
Outreach Services
1:1/2:1 support to access the community and undertake social activities e.g going to gym, attending social groups, accessing work and/or education.
Domiciliary Services
Support with personal hygiene, meal preparation, medication, and maintaining the home environment.
24 live in care
For young people who require 24 hour care or support within their own homes either on a permanent or respite basis including emergency respite
